We could use your help. Our site has been up and running for several
months and we would like to get your feedback. We would like to get your
answers to the following questions and as a reward, we will include a $10 bill
in your first order no matter what size your order is! To participate, just
fill out the form below and when you place your first order, we will include a
$10 bill in your shipment. We appreciate your feedback and feel that you
should be rewarded for helping us out. When you fill out your first order, please be
sure that the email address on your order form matches the email address that
you give us in the survey below. This is how we will know to send you your
